If you are over 18 and have think you may have an eating disorder you may be able to obtain a referral to our service:
- Via your GP (if your GP is based in any of the areas on the map below)
- Via secondary mental health services (e.g. CMHT) or other organisation (e.g. First Steps, FREED, Beeches).
A referral form should be completed and returned to us by email to dhcft.eatingdisordersservice@nhs.net or posted to Unity Mill, Derwent Street, Belper, Derbyshire, DE56 1WN.
