How to make a referral

Referrals are accepted from health, social care, educational or third sector professionals. To make a referral, please download and complete the forms below. Please supply as much information as possible so that we can ensure that the assessment is carried out by the most appropriate professional(s). Referrals will not be processed without the completed questionnaires. Please send all forms together.

Before making a referral

We are currently receiving high volumes of referrals some of which are not appropriate and this is having an impact on waiting times.

Before making a referral, please consider whether other forms of support would be more appropriate than a neurodevelopmental assessment such as an early help assessment, parenting course or referral to other agencies. Please see our list of useful links for other avenues of support.

Please ensure the referrer or keyworker has known the child/young person for at least 3 months prior to referral, otherwise the referral may not be accepted. Working with the child/young person for a minimum of three months will ensure you are able to provide sufficient information for the referral.

Please note, the Trust does not assess for ADHD in adults, nor does it provide medication monitoring or post-diagnostic support for ADHD in adults. Please contact Sheffield neurodevelopmental service for more information about assessment for ADHD in adults. If you are assessed by the Sheffield team to have ADHD, your GP will be expected to sign a shared-care agreement to take responsibility for monitoring your medication needs.
