Referrals onto the neurodevelopmental pathway are assessed by a multi-professional team as part of the Children's Services Single Point of Access (SPOA). The neurodevelopmental pathway and SPOA are a collaboration between organisations including Derbyshire Healthcare NHS Foundation Trust, University Hospitals of Derby and Burton NHS Foundation Trust, NHS Derby and Derbyshire Clinical Commissioning Group and Derby City Council.
Before referring, please consider whether other forms of support would be more appropriate than a neurodevelopmental assessment. This may include referral for parenting courses, early help or MAT referral, and support with learning needs (for example through the SEND graduated approach).
Please provide evidence of a graduated response to support the child/young person’s needs as, without this, the referral may not be considered.

Who can make a referral?
Referrals are accepted from any health, social care/MAT, educational (SENCO) or third sector service by downloading and completing the forms on the pathway referral page.
Please supply as much information as possible so that we can ensure that the assessment is carried out by the most appropriate professional(s). Referrals will not be processed without the completed questionnaires.
