Welcome and thank you for visiting our site, our team works with children and young people aged 5-19 who have an intellectual disability. We offer support to parents/carers and professionals in relation to health and wellbeing needs that cannot be met by universal services. 

Accessing the service

To access the service the child must be registered with a GP within Derby City or Southern Derbyshire. Referrals can be made by any professional and are discussed at the SPOE (Single Point of Entry) meeting with our colleagues from the CAMHS Intellectual Disability Team and the Complex Behaviour Service, to determine the best service to meet the child’s needs at the time.

We are a small team of registered nurses, learning disability nurses, children’s nurses and a support worker. Once accepted into our service we complete an initial assessment with parents/carers to gather more information about the referral and presenting needs and agree a plan together based on parents/carers priorities for support.

With parents’ consent we may liaise with other professionals involved in their child’s care and observe their child either at home or in school if appropriate. We may see the child/young person and their parents/carers in home, clinics, or workshop settings.

What support is offered?

A programme or workshop:

  • Toileting workshop
  • Behaviour workshop
  • Cygnet programme

Specialist assessment/intervention:

  • Behaviour management
  • Health promotion - sleep clinics, eating and drinking needs, a physical health need, puberty/growing up/sexual health
  • Life skills - continence/toileting, personal care/hygiene, relationships with others, keeping safe, school life, leisure time, emotional intelligence, communication, decision making, routines and transitions, loss and bereavement.

The support we can offer is time limited. We require parents to be motivated to make changes with our support to enable good outcomes. Care plans and progress are reviewed with parents throughout our involvement.
