Guidelines for referral to TMHD

Talking Mental Health Derbyshire fulfils national Improving Access to Psychological Therapy (IAPT) guidelines, by offering short term, NICE approved therapies at step 2 (guided self help) and step 3 (Cognitive Behavioural Therapy, Counselling for Depression, Interpersonal Therapy, Dynamic Interpersonal Therapy, Eye Movement Desensitization and Reprocessing).  We endeavour to help as many clients as possible, but in order to do so, we must ensure that clients who have a higher level of need than we can serve are quickly identified and action taken.

IAPT clients must have problems of mild to moderate severity, with no serious risk issues either current or within the last 3 months, be motivated for treatment and able to engage with short term psychotherapy and tolerate the increase in affect likely to occur within therapy.  We can only expedite cases where the client is a veteran, or pregnant or the mother of a child under 1. We do not conduct urgent sessions. Clients may self refer or be referred by health professionals. Barriers to accessing TMHD services

Incidents of violence to others, self harm or suicide within the last 3 months.

Problem drinking or drug use within the last 3 months.      

Suicide attempt or serious self harm in the last 3 months.

Open secondary care services, with the exception of clients who receive medication management only. This includes concurrent referrals to secondary or specialist care services.     

Clients with dementia.  

Clients who have been deemed unsuitable for TMHD within the last 3 months unless there has been a prior discussion between services and a rationale for this re-referral.

Clients who have had 2 episodes within the same treatment modality within the last year for the same problem. 

Clients who have not attended the last 2 appointments offered with no notice given.      

 

We request that professionals make referrals to us directly, rather than giving out our leaflets. We will liaise with you to discuss any inappropriate referrals and In these cases, we will ask you to contact the client to inform them of this fact. If we have not been able to reach you after several attempts, we will close the referral and send you a letter informing you of this fact.
